Medical Records

Services include medical in- and out-processing, release of Information, medical record copy requests, and medical records repository. Patients must be 18 years or older to request a copy of medical records. Spouses can only receive copied records if a release has been signed. Please visit our department for instructions regarding release of medical information. Radiology and Dental Records must be requested from respective departments. If you have medical records from off-base providers that need to be entered into the 35th Medical Group system, you can come to Medical Records during office hours. Records on discs need to be printed out.
